Data types and usage There are two types of data necessary in determining an informed community assessment. Primary and secondary data. Primary data is data that is not available on record and can only be obtained as a first-hand source from the community members and the community environment. Secondary data is data that is collected and analyzed by other entities for other health indicators and can be retrieved to provide information on a community. Such data can be obtained from government health information departments such as community indicators of health such as infant mortality data, maternal mortality, disease burdens among others. Determinants of Health Determinants of health are factors that influence the health and wellness of a community (World Health Organization {WHO}, 2018).

The determinants of health data can vary from community to another, it represents data that is integral in determining the health seeking habits of the community members, indicates the possible reasons for health issues and provides information on which service gaps exist in the community (Adler, Glymour & Fielding, 2016). The determinants of health include income and social-economic factors such as community members employment and working conditions in the community. Income and social economic status contribute to the health needs and ability to access the healthcare services. Research shows that an improvement in social or economic status is associated with improvement in health outcomes (Adler, Glymour & Fielding, 2016). I will then engage the community members and leaders and health stakeholders in the community to share the needs and aims of the data collection.

To prompt community response, I will utilize the secondary data to inform the community of existing gaps in health. Being an underserved community, I will depend mostly on focused group discussions and observations to collect primary data as it will provide a more personalized contact with both the community and their physical environments. Observation will provide data on the physical environment, social and economic status and behavior including cultural practices while focus discussions will be essential in collecting data on health determinants and clarifying the data collected through observation.
