Most of the chronic conditions such as congenital heart disease (CHD) require a lot of care and thus such children are kept in pediatric centers for some time. After leaving the centers, they are either discharge or transferred to adult centers where the services are much different. Young adults with chronic conditions such as CHD do not receive the necessary cardiovascular services at such centers. This raises the issue of how young adolescents should be prepared before they are transferred to adult centers or discharged as well as how the conditions can be property handled at that early stage. Adolescents have the fear of been seen as coming from pediatric centers. The goals ensure that patients receive appropriate care that is necessary for their recovery (Rivers et al 1370).
