The population observed at and around BCC are low income and homeless adults with and without disabilities. Identifying the communities most vulnerable people initiates the investigation for the community health nurse (CHN) to collect necessary data and information to build onto the community’s goals for improving health outcomes in this population. Working to meet community health goals, the CHN includes their understanding of social determinants of health to seek ways to improve the community’s environment, access to food, housing, health and healthcare resources. Planning for interventions to improved access to healthcare for low income and disabilities included the overarching goal to reduce further health disparity within this population. The community thus exhibits, positive growth of industry and economic development.

Many health issues are evident amongst the homeless population, and some concerns include communicable diseases, drug use by injections as needles can be seen on the walk paths and stairwells that the public use. A good number of increased cases of persons with disability can also be seen as deduced from the number of people using wheelchairs as there are cases of amputations. Mental health such as paranoia and hallucinations were also noted amongst the homeless, and they can be seen operating in groups like they have their families while others could be seen with dogs as companions. Of significance is the central city concern which operates as a nonprofit organization with a primary basis of assisting the homeless by giving them resources for employment, offering housing, access, and benefit to health and recovery.

PICOT Question The PICO question was formed to assist in finding relevant information, such as being useful in various databases and search engines (Melynk & Fineout-Overhault, 2011). The final results were presented in a summative manner from various literature. The purpose of the research is to break barriers preventing people living with a disability (PWD) from accessing healthcare services while using effective communication in creating awareness. The research also illuminates at the various barriers which are listed as a negative attitude by both the public and healthcare providers, ineffective communication, ethnicity, level of income, and insurance coverage (Sharby et al. The article also discusses the various, means of curbing those barriers. The articles also discuss challenges faced by care providers in their bid to assist the disabled population.

The strengths and limitations of the research are discussed briefly, and one strength of the research is getting to analyze a number of studies applying different study designs to come up with a summary. One limitation identified is time constraint. The recommendation made is based on the study. For instance, the author advocates for a more effective communication strategy that is patient-centered and would lead to client satisfaction, this will address the issue of PWD failing to attend appointments due to previous experience of miscommunication and disrespect by care providers.
