A follow up was conducted on the following day between 2 pm and 4 pm after lunch. The client is a 63-year-old Jamaican male. He resides with his wife of 40 years. Their two daughters from their marriage live nearby. The client is Afro-Caribbean ad a Christian, attending the Roman Catholic Church. Family characteristics Persons living in Household Client XX expresses joy in his family who are almost ever present. His wife is always with him at home, helping him with various needs such as taking medication and providing meals. This helps him adhere to a strict medication protocol. XX’s two daughters are always present at home during the weekends and occasionally visit during the weekdays in the evening when XX is back from work.

When they do not make it to come home, they call to check on their parents. He also state that he derives much inspiration from his family and the sight of his happy family. Extra-personal strengths include the support of his family and colleagues at work. This makes him feel loved and gives him the energy to fight the problem in his life, which include the two chronic health conditions. He states that his family has been his source of strength due to the love that they afford him. He is grateful to his wife for being so supportive especially when he goes through health scares such as the realization that he has ESRD. He does not have sex outside his marriage as it goes against the catholic doctrines.

Sleeping arrangement XX lives in a 4-bedroom house with his wife. The house also has a kitchen and a living area. It also has a garage to the outside. He sleeps with his wife in one of the bedroom. His eye pupils responded well to light. He has good eyesight and does not need glasses. He had a relatively normal heart rate without murmurs. He had no form of oedema. He also had normal bowel movements, preferring to go to empty his bowels in the morning after waking up. He is in close contact with other veterans he served with. He describes some of his most heroic moments in the military and the wars he participated in. However, he says that his most important concern at the moment is providing for his wife and being available when his daughters need him.

He is also glad that his daughters are now responsible adults who do not need his supervision anymore. He is greatly respected in his household and is consulted on matters pertaining to the family or the household. XX has an insurance cover, which he finds beneficial to him when he needs medical intervention. Ethnic background Religious affiliations XX identifies as a black male with a Christian background. He was raised in a Catholic family and has held on to that faith to the present day. He attends the church on Sundays with his family. He is an active participant in the church, being a member of the church choir. Upon waking up, he does his own exercise regimen which includes doing about ten push ups, ten sit ups, and five pull ups.

However, he does one of these each day and not all of them in a day. He then walks out of the house in warm clothing to walk or jog depending on how he feels like on that particular day. He is out for about 10 t 15 minutes before returning to prepare for work. He takes a hot shower and haves breakfast later on. XX does not like bothering his wife or his daughters when he needs with something. However, he states that they show him so much love that he is not afraid to ask for help with problems. His daughters are so understanding, according to his own narration, and they encourage him to remain strong and committed to good health.

Medical diagnosis XX seeks medical advice and check-ups from his primary care physician every time he goes for haemodialysis. Part of the advice includes changes in medication and discussing side effects of the drugs he uses to control hypertension. XX is a Catholic and drinking is not an issue according to his faith, as long as he does not drink to be a problem to his family or the society. However, this is an indication of poor coping mechanism since his condition disqualifies him from taking alcohol. This is an indication of the need to have patient education and nursing intervention to help him reduce alcohol intake. Two major problems and nursing interventions XX s alcoholic and needs to seek the help of a professional.

He could also get assistance from a Rehabilitation Centre. A recommendations to his primary care provider should be made to enable them discuss this. This will be important in ensuring that the patients makes better health decisions. A transplant could save him many days of excruciating dialysis, which he finds to be “draining and sometimes painful”. This will also make him make attempts to get a donor. It is critical that he gets on the list as soon as possible. For instance, he will be able to overcome alcoholism. He undertook AA meeting to help resolve his alcoholism. This has helped reduce his alcohol intake and he intends to stop drinking completely to avoid “complications” as he cites. After our first discussion, it appears that he attempted to seek more information about getting on the donors list by visiting the resources directory.
