Thus, this business plan is about an advanced care directive that individuals will have to fill, and this will be used as a reference for any medical decision that will be made during their end-of-life stage. Product Definition The local geriatric office offers health care services to the old populace. Some of the services being offered include; preventive care, fall prevention, and diabetes management. Additionally, the office also manages heart diseases such as coronary artery disease and hypertension. It also treats kidney conditions such as female urinary incontinence, male urinary dysfunction, and overactive bladder, among others. Thus, patients visiting will not receive medical care only but will also get ACD services. 3% of Americans had completed their ACDs while 73. 7% did not. However, as time goes by, the level of education among people all over is rising significantly as the poverty level goes down. As a result, more people are gaining knowledge of the importance of completing an ACD.

Based on a research done by Bisognano and Goodman (2013), more people above 30 years are expected to embrace the culture of competing an ACD. The opportunities include increased awareness of ACDs by the Americans, a good integrated IT system that can capture patients’ data for a long duration, presence of clinicians who are willing to learn about ACDs, lack of ACDs services in competing centers, and a proposed legislation of payment of doctors who educate patients on ACDs by Medicare. The threats include the uncertainty of whether ACDs services are legal in some states and unwillingness to complete the ACDs by patients. Rough Financial Plan The business plan implementation will be key in the organization. For a proper implementation to be achieved, a few changes have to be introduced.

Firstly, there need to be an education forum where all the healthcare providers in the center are trained on ACDs and its benefits. Since the health care practitioners will be educated on the subject, they will be at the forefront to spearhead the project to patients. However, completion of ACDs will reduce patient stay to approximately 4 months (Sævareid, Lillemoen, Thoresen, Førde, Gjerberg, & Pedersen, 2018). This will increase enable different patients to use the same facility and also decrease hospitals bills incurred by the patient. Basically, the decision making will not be a difficult as before, where families could become stranded in selecting the best decision, that’s affordable, and that which will ensure their loved one becomes well again (Sævareid, Lillemoen, Thoresen, Førde, Gjerberg, & Pedersen, 2018). In case of a patient’s health crisis, the ACS will be retrieved and the desire of the patient followed to the latter.
