Projections are that close to 8 million adults will be living with chronic heart failure in 2030. Such a number is alarming and calls for necessary preparations. According to Medicare, the risk for hospitalization for CHF patients stands at 23%. Readmission is perceived by medical researchers as a correctable source of the low-quality car and excessive medical costs (Shaffer & Maurer, 2015). Prevention of readmission is a step towards less spending of medical costs, increasing safety on patients, and promoting quality of healthcare. Heart failure patients will be the direct beneficiaries of the implementation of this evidence-based strategy because of improved safety and likelihood of quality outcomes (Whitten, 2013). Health practitioners and heart specialists will also find an easy way of preventing readmission. The fact that it has also been found to be cost-effective will save healthcare organizations and CHF patients from unnecessary spending (Yawn, 2013).
