The environment within which a community lives is a key predictor of the health outcomes of such groups of people (Braveman & Gottlieb, 2014). Actions of the community members may work towards improving or reversing the health of the community members. A windshield survey involves driving through the community observing the community’s infrastructure such as the parks and recreations, community healthcare facilities, the environment, transport system, security within the community among other aspects of the community function. The area that was Santa Ana California. The culmination of the windshield survey was to identify healthcare issues that were within the community. Grass are not well maintained. The area along the Euclid street towards McFadden avenue is lined with well-organized housing units consisting of bungalows on either side.

The housing was well organized and fitting for family occupation. The housing units were well planned with adequate spaces and connecting lanes within the neighborhoods. The neighborhoods are well planned with business centers which had a supermarket (Mom’s supermarket) and a sea-food restaurant. There are Women, Infants and Children’s (WIC) clinics in the 17th street, 1701 S Grand Ave, 1725 W 17th St, 5015 W Edinger Ave, 631 S Main St b. The WIC programmed supports low-income women infants and children under five providing access to healthcare, foods and referrals. Other free clinics in the areas include Her Choice Women’s clinic, Orange County Needle Exchange Program and An Lanh free clinic. Housing In residential area specifically mohawk drive. Houses are painted with bright colors with multiple cars parked outside the house with some gates are also broken.

There are parks in the city, the Centennial regional park 3000 west Edinger avenue, Santa Ana ca 92704. Area is dirty and smelly and have some homeless people in there. Families are having picnics next to the manmade lake which is very dirty with full of ducks’ water is also greenish and moldy. Santa Ana Parks Recreation. Social services center The Cal fresh office is located North of the Centennial regional park helps low-income individuals to access nutritious diet on their budgets. Other notable eateries in the area include, Panda Bowl, Sizzler, Lee’s sandwiches and a deli at the Stater Bros. markets which the time of the survey was busy serving its clientele. Markets, Mom’s supermarket among others. Mexican foods are sold at the Gonzalez market. Barnabas orthodox church, Episcopal-church -The messiah among others. Many catholic churches as well were noted in the areas such as Our Lady of the Pillar church on the 6th street, St. Joseph church on Minter street, Our Lady Guadalupe Delhi on the Central avenue, La Purisma Catholic church among many others. The number of catholic churches are relatively more than protestants indicating most of the population are Catholics. The church compounds are well kept most having well-organized parking slots. Traffic from motor vehicles in the residential areas is minimal an indication that most residents prefer using the bus or subway to work. 1  Total housing units 73,174 Homeowner vacancy 1. 9% Rental vacancy rate 4. 9 Median Household Income $53,335 Individuals below poverty level 21. The rate of poverty in Santa Ana is an estimate 21. 2 %, consequently the health insurance cover is low with 25. 8 % of the population below 65 years lacking health insurance. These coupled with the likely language barriers and low health literacy among the immigrant population (who form over 40% of the population) increases their risk for contracting lifestyle illnesses. The high numbers of fast foods and poor uptake of exercise regimes further increase this risk (Tung & McDonough, 2014). Children play areas were empty with most of them using skate boards or bicycles from school. This reduces the children’s exercise which increases their risks for obesity. The unsanitary conditions observed at The Centennial Regional Park poses a risk of disease outbreaks such as Cholera.

The park poses more risks for the people living near the park as well as those visiting for picnics. We also noted some homeless people living near the park which poses some risks for nutritional deprivation. As community health nurses, we will also engage the community’s opinion leaders to advocate for improved exercise and encourage balanced dietary intakes (Sisnowski, Street & Merlin, 2017). Through community activities and events, we will share healthy messages and involve the members of the community to encourage community ownership and acceptance of the risk. We will work to leverage on community functions and gatherings to pass on information. The establishment of awareness materials such as bands, flyers and labelled T-shirts are critical in passing on the message on healthy eating and safe exercise.
