Such kind of research is essential in the identification of evidence-based practices that can be incorporated as standard procedures in the delivery of healthcare. For this reason, the chosen project attempts to evaluate the outcome of the psychiatric patients that receive their medications compared to those who do not. In this paper, the strategies to create change will be discussed with a description of the implementation’s stakeholders. Strategies to promote engagement of the implementation will be addressed. A barrier of the project will be assessed, and methods of elimination will be evaluated. The communication can be done through the patient’s electronic medical record and or any organizational approved technological device. This can allow all the stakeholders; such as the social worker, pharmacist, doctors, nurses and the organization’s management to agree on the same goal.

The utilization of technology will allow all that is involved in communicating effectively and having access to large amount of patient’s healthcare information, at any time and from anywhere. The second intervention that the advanced nurse can implement is to educate the professionals involved in the care about the importance of preventing the readmissions and strategies that can be done to prevent the readmissions. Examples of these strategies include properly identifying these patients, importance of effective communication between the team and how the identified problem can cause a rippling effect on the cost of healthcare. To properly assess the barriers, the advanced nurse must know how to identify the barriers. It is imperative that one looks for the stakeholders that are not adopting the change and or those who do not feel like they do not have the of support from their leaders.

Additionally, poor leadership skills can cause a lack of interest of the stakeholders. Lastly, one must assess those Leaders that lack vision, do not lead by example or give the staff any feedback. Elimination of Barriers To eliminate these barriers, the advance nurse leader must attain good communication, listening, and critical thinking skills. Electronic medical records and other electronic outcome measuring tools can be used to compare and measure the outcomes of the patients that receive the medications before discharge and the ones that didn’t receive the medications, against their hospital admissions. Conclusion The advanced nurse is more than prepared to assume a leadership role that can create a change. Effective leadership is a significant aspect that can foster an organization to achieve its objectives.
