As a group, consider the following questions whilst developing your health promotion program. Think about the ‘Who, Why, Where, How, What & When’ whilst making your decisions. Remember to also utilise the information from LGA health & Wellbeing Plans and Priority areas of Local, State or Federal governments. Question 1: What evidence do you have from your community to specify the health issue and focus population for this program? (What data tells you there is a problem here? Who is at risk?) Asthma has been considered as one of the leading chronic diseases for children and especially between ages 1-14 years.

, already addressing this issue in this community? If so, what are they and are they being effective? (What is already being done? What gaps are there?) Asthma Policy drafted in conjunction with The Asthma Foundation of Victoria provides for a deliberate policy and guidelines that are present in the Maribyrnong area towards helping address the issue. The policy is mandated to help provide a safe and healthy environment for the people in the region. The policy has been guaranteed under some of the following programs: Approved Emergency Asthma Management Training; Asthma Child and Adolescent Program; Asthma Friendly Children Services Program; and Asthma Action Plan, (Maribynong kindergarten). All these help address the issue of Asthma in the area through the Asthma policy.

Question 4: What resources, assets or strengths does your community have? (What can you use from WITHINYOURCOMMUNITY to help support your program?) The community under the new government program to help uplift the livelihood of people in discriminated areas on government services has been focused. These groups are conversant with the situation and will help pass on the information so as to take necessary precautions preventing the happening. Question 2: What are the objectives that relate to your goal? How will you realise these? – relates to impact evaluation (What changes do you need to make to achieve the goal?) Approach on new therapeutic approaches is one of the changes I would like to initiate within the healthcare of the Maribyrnong community. Currently, access to healthcare consultations is not prevalent among the population.

The new therapeutic approach aims to bring on board continued consultative and frequent visits to healthcare facilities by the mothers and their children, (Martinez, 2009). Under this, it is likely that there will be mitigated efforts on the condition. It will look at the framework you have based your processes on, the ‘Who, Where, When and Why’ factors that you need to address when presenting or delivering your program. You will need to consider the decisions being made as to how you wish to make the best impact through your program design. Also at homes of selected people will be necessary to provide for one on one interaction moment. All these localities are selected due to the direct interaction and efficient one with the population towards communicating the information. Question 3: What individual health education (or personal skills) need to be considered? What knowledge deficits do you need to address? (What education is needed or gap in knowledge recognised? For Whom? Why?) Asthma as a chronic condition needs to get the proper attention from the professional personnel other than the provided assumption by non-experienced population. Clinicians and physician education is first the most needed knowledge and educational background. Think about how you might continuously reflect on the processes being under taken in order to gather information and make your decisions. Consider the challenges you have to face to measure the impact of your program.

Think about the strategies you have chosen in order to reach your goal, are they appropriate? How do you know? What changes may have been necessary along the way? Question 1: Outcome Evaluation How will you know that you have reached your SMART goal? (What tells you your intended outcome has been achieved?) In checking the achievement and progress of the intended goals, the reflection about reported cases of asthma among the hospital facilities will reflect if the awareness program has been successful. Within the duration of one year after the initiative, it will be enough to get the medical reports from the institutions and reference the cases with the previous years. Counter checking the medical report will offer a designated answer towards knowing and establishing the aspect of success on the program of awareness creation.
