Symptom/ Presenting evidence (as evidenced by) NOTE: Each nursing diagnosis should be one sentence that contains all three components (Nanda statement / related to / as evidenced by) Actual Nursing Diagnosis The patient has been diagnosed with stress overload which has been caused by the innumerable commitments that have her pressured as evidenced by the difficulty she has concentrating to get all things done concurrently. Potential Nursing Diagnosis (risk of) The patient has been diagnosed with the risk of ineffective activity planning related to her schedule that is inundated with countless responsibilities as evidenced by the number of pending tasks. Nursing Care Plan Priority Nursing Diagnosis with rationale • Stress overload has been prioritized as the actual nursing diagnosis that demands urgent attention because of its implications on the patient.

Evidently, it has been selected as a priority because the patient has demonstrated anxiety, tension from external pressure, and problems concerned with decision making, all of which are considered to be signs and symptoms of stress overload (Lunney, 2006). In the event the nursing diagnosis is not treated, the patient might end up being a victim of other stress related illnesses which form 90% of all the chronic maladies (Burdick, 2014) Client-Centred Outcome Write one goal/ outcome statement that meets the SMART criteria SMART criteria: • Specific • Measurable • Attainable • Realistic • Time-based All SMART criteria in one sentence • The patient will participate in eradicating stressors in her daily routine and point out three interventions that can be employed to reduce her stress overload by 30th of November.

Besides, sleep has been proven to statistically ameliorate the academic performance of students (Eliasson, 2010). Physical activity is efficient in the reduction of stress because it helps the patient shift focus from her mind to her body, increases rate of blow, and supplies more oxygen to the brain. Consequently, this produces the endorphin hormone which suppresses the cortisol hormone hereby neutralizing stress (Pletcher, 2012). Summary Introduction The Nursing Process is a step by step iterative procedure that is used by registered nurses in the medical field to develop a program that suffices to identify the health issues in a patient, provide remedies, and monitor the outcome of the patient. Without a doubt, the nursing process requires critical thinking as different patients come with different medical conditions that demand different approaches.

In this research, a basic interview was conducted comprehensively and from that, a clear diagnosis was formed from the information concerning the patient’s life. The assessment phase is highly significant because the information collected herein forms the basis of the entire nursing process. Diagnosing Second, nursing diagnosis follows after an assessment with the intention of hypothetically establishing the root cause of the patient’s health problem. According to (NANDA International, 2012), nursing diagnosis is the clinical judgment about the patient’s experiences or responses towards an actual or a potential health problem. With regards to the information collected in the assessment phase, a client can have multiple diagnoses, both actual and potential. Implementation Next, the doing stage arrives where all that has been said and consented upon by the patient and the nurse is implementaed.

Here, the nurse follows to execute the nursing care plan, taking into full consideration the specific, attainable, and realistic goal that was set. It is in this stage that nursing interventions are seen whereby the nurse uses the medical expertise to help the patient psychologically, emotionally, physically, and spiritually where possible. In our case, we assisted the patient by educating her on the importance of sleep, teaching her the basic physical activities that could be utilized to keep her mind revitalized, and guiding her on how to properly manage time to ensure she manages to juggle between all the important facets of her life. Indeed, the implentation stage is important because this is the phase that demands ultra precision because every action in the nursing process is undertaken in this stage.

Concept of Person The person in this concept is defined as the patient or client who is considered to be the primary recipient of all the nursing care in question (Thompson, 2017). Majorly, a person in the nursing process is one individual even though the term can be overarching enough to cover the patient’s family in some cases. Nonetheless, the concept of person was befitting our roommate for she was the recipient of all our nursing care. As a result, we worked together with her in determining the desired outcome and pursuing them tirelessly. Concept of Health The concept of health can be classified under relativism because the definition of health differs from one person to the other, in that what a person considers healthy may downright unhealthy from the perspective of another (Thompson, 2017).

Hence, social determinants of health can be any external conditioning ranging from the economy, politics, transportation, residential neighborhood, or access to resources that form the root causes of health problems in the society(Demaio, 2012). In our scenario, the main social determinants of health directly affecting our nursing care on our patient are gender, economic stability, and social support system. Gender Men and women have different responses when it comes to health and diseases. Upon a research done by (Kennedy, 2016), the top diseases and health problems affecting women are alcoholism, heart diseases, mental health, sexually transmitted illnesses, osteoarthritis, and stress. Considering stress has been noted as a top cause for death in women, it directly implies that our patient was not undergoing a casual matter, rather, a matter of exigency.

Moreover, her busy schedule from the clinic to the chain store, back for an evening class, and later in the night to work on a scholarly article did not allow her to sit around with friends even for a casual laughter. Doubtlessly, her officious life and other unavoidable circumstances pushed her headfirst into the stress overload realm. Hence, we proposed that she find time to hang out with friends for moral, social, emotional, and psychological support. Conclusion In summary, the nursing process is a crucial element in the administering of nursing care to patients in this time and age. In fact, one cannot be considered to be a qualified nurse if they do not have the requisite know-how of conducting the nursing process.
