The team had met following the signing of his discharge orders to discuss his discharge care planning. The report will aim to explore the barriers of interpersonal communication identified of the team, the level of collaboration between the team members the impact they have on the patient, the organization and the providers themselves and interventions that can be put in place to improve interpersonal communication of the interprofessional care team.

There were no limitations to the achievement of the aims of this report. Healthcare environments impact on the patient outcomes as well as providing performance. Effective healthcare delivery environment can be ensured through practicing effective team communication between the health care providers through employing essential skills such as effective non-verbal communication, timely feedback among others. Interpersonal communication skills enable healthcare providers to organize their work and responsibilities in the care of the patient. Through effective planning, there are reduced adverse patient outcomes, care left undone for the patient, reduced medication errors and also reduced healthcare staff burnout and turnover (Lee & Doran, 2017). Interpersonal communication skills such as collaborative or two-way communications are critical to developing the capacity of new healthcare providers and sharing knowledge.

Effective and active listening and employing positive non-verbal cues such as nodding in agreement, avoiding fiddling and fidgeting during the discussion and maintaining eye contact with the speaker. Avoiding the use of mobile phone during communication is also critical and shows interest. The team ought to also avoid side talk and ensure that delivery of information is done in a factual and sensitive manner to avoid conflicts among team members. Analysis of the effectiveness of this inter-professional team Interprofessional team members are critical to the patient care provision and outcomes. The collaboration among the interprofessional team members is crucial in ensuring effective coordination of care activities as well as role placements for the care teams (Busari, Moll & Duits, 2017). In the scenario, the patient discharge has already been signed yet most of the patient education is yet to be done as evidenced by the care team members who keep indicating that “someone needs to talk to him about….

” Val Palumbo, (2013) and an assertation by the nurse that the discharge counselling is yet to be done. It is also noticeable that from the patient’s presentation, the interprofessional team completely fails to include a dietician to provide input on the patient’s dietary requirements. Effectively composed interprofessional teams lead to effective patient care interventions which are critical to ensuring patient-centered care delivery. Organizational impact from such a team performance Interprofessional team collaboration and effective communication is critical to ensuring that the delivery of patient-centered care is done so effectively. Individually communication training and sensitization is a key recommendation. Training the healthcare providers should aim at entrenching effective utilization of interpersonal skills in their communication and training on effective non-verbal communication.

Individuals must also regularly practice the utilization of key interpersonal communication skills in their delivery of health care needs for the patients. Mentorship programs for the providers on effective interpersonal communication are also imperative to achieving effective interpersonal communication within the organization.
